- Description: Summary: -Neurosurgery/Trauma ICU management of emergency surgery patients that are brain neck and spine surgeries. NP with neuro-trauma and/or neuro-critical care background/experience Rate Caps for Submission:Malpractice Must be included in rates.Orientation - half the hourly/callback rate kicks in after 8 hours. Per hour oforientation or training beyond 8 hours Hourly 170-195 - Per hour not OT Holiday - Must be 1.5x the hourly rate. Per holiday hour no OT Schedule/Availability Requirements:Updated dates: Jan 8-9 Jan 14-16 Jan 19-28 Feb 3-25 March 3-4 March 9-18 March 23-29 April 7-12 April 20-26 May 4-13 May 18-27 June 1-7 June 15-21 June 29-Dec 31Facility Location: Redding CASetting: Level II Trauma CenterFTE:Duties:Required Procedures: Low/moderate risk procedures such as laceration repair IV access/IV insertion-peripheral line fracture and dislocation reductions minimal/moderate displacement. High risk procedures: G-tube placement endotracheal intubation cardioversion/defibrillation LMA paracentesis chest tubes central and arterial line placement.EMR: Cerner Care BSupport Staff:Reason for Coverage: Gaps in coverage Travel Preferences:Preferred Lodging: Please see attached must align with Travel PolicyAir Travel: Local no flights? PreferredPlease detail any anticipated travel exceptions for example a dietary restriction necessitating specific lodging situations. Travel exception requests will be denied if this information is not part of the provider presentation. Credentialing Information:Timeframe: 30-60 days from receipt of FULLY completed applicationRequirements: Must be on active staff at a TJC accredited facility with unrestricted privileges NOT just working as a locums with privilegesCredentialing with other facilities? NoBilling enrollment - Is this position providing services in lieu of an existing enrolled provider for a period of 60 days or less?: No Additional Details / Questions Answered by Facility:Average Number of ICU Patients per Provider per day: 15-20 patients on service per day MSO requires the application completed within 3 days of being confirmed despite the start date!
