Job Summary
The Department of Obstetrics and Gynecology at SUNY Downstate Health Sciences University is seeking a full-time Social Worker III. The successful candidate will:
- Provide psychological assessment, support and initiate patient referrals for proper management.
- Identify the psychological needs of the patients and to ensure the patients' needs and services are addressed.
- Collaborate with the Attending Physicians in the clinics for appropriate patient follow-ups.
- The overseer of the MICI Program staff and the logistical details.
- Functions collaboratively with the MICI staff on a daily basis for patient related updates.
- The proper management of the Providers' referrals and the completion of the required MICI forms.
- Provide the appropriate follow-up of the high risk patients for compliance measures.
- Provide the patients' documentation in the HealthBridge system.
- New York State Licensed Master Social Worker (LMSW) or Licensed Certified Social Worker (LCSW).
- Excellent communicator with a willingness to collaborate with interdepartmental, institutional and community partners.
- 1 to 3-years of post-masters experience in a hospital or health care setting.
- Acquired experience with the maternal and infant population.
Monday to Friday; 8:00am to 4:00pm (Full-Time)
