Lead a team of Advanced Practice Providers and collaborate with an interdisciplinary team, including Psychiatry and Psychotherapy, Podiatry and Wound Care (where available). Detailed Duties and Responsibilities

Provide primary-care services in accordance with Eventus WholeHealth site-specific protocols and the standard of care for family medicine, geriatric medicine, and/or internal medicine as it pertains to long-term care facility work. Review and sign MARs as requested by facilities; sign all orders specific for a supervising physician while at the facility and/or within 24 hours if faxed or emailed; address labs within 24 hours of receipt; manage Scheduled Medication prescriptions per facility policies. Provide facility with adequate time during each visit for paperwork and administrative duties. Work with facilities to manage systems; attend care plan meetings and QA/PI (Quality Assurance/Performance Improvement) meetings as requested. On Call: Providers are expected to be available to their facilities between 8:00 AM and 5:00 PM on scheduled days; triage and on-call duties follow the latest schedule. Submit copies of documentation and billing records to Eventus WholeHealth according to billing schedules. Maintain open and positive communications with facility staff and administration; provide instructions or feedback regarding medications and other pertinent caregiver information; meet with Administrator or designated facility care coordinator prior to and after patient visits each day. Be knowledgeable of and adhere to Eventus Whole Health standards, policies, procedures, and all applicable laws and guidelines; comply with regulatory agencies governing health care delivery. Maintain patient confidentiality at all times; comply with HIPAA rules and CPT coding guidelines. Qualifications/Education

Licensed MD or DO 3 years in Long Term Care/Geriatric Care preferred AMDA Certified a plus but not mandatory Strong leadership qualities Strong organizational skills Excellent written and verbal communications skills About Eventus WholeHealth
