Job Summary

Under the direction of a Registered Nurse and the medical orders of the Practitioner, provides direct and indirect nursing care in an outpatient setting.

Essential Responsibilities

Reviews and prepares medical records for assigned patients; initiates follow-up as requested; reviews patient needs; identifies and prepares orders for authorization by practitioner and may accept verbal orders per policy; observes, collects, and documents new information; informs RN or practitioners regarding incomplete data, new observations or abnormal/unusual data.

Implements basic nursing procedures per plan of care; administers treatments, immunizations, and medications as prescribed by practitioner; prepares medications for practitioner use; performs point‑of‑care testing as authorized; provides laboratory, x‑ray, and other reports to practitioner; notifies patients of normal results; considers special needs of different age groups; sets priorities of care based on acuity.

Documents implementation of orders/interventions; records findings, instructions, and patient responses on appropriate forms or online records using department and/or JCAHO standards; observes patient/family responses; reports unexpected responses to RN or practitioners; collaborates to revise care plan; demonstrates competency in basic nursing skills and equipment; maintains current knowledge and skills; attends and participates in staff meetings and projects.

Performs efficiently in emergency patient care/clinic situations; follows established procedures; informs appropriate practitioner/RN; assigns basic nursing tasks to unlicensed personnel as appropriate and according to standards; provides clear directions; acts as resource to unlicensed staff; follows up to ensure care is given per standards, policies, procedures, protocols, and guidelines.

Follows discharge plan developed by practitioner or RN; processes and/or follows up with referrals; provides instruction and pre‑printed information to patients; observes and documents patient/family response; reports concerns.

Manages all patient vaccine data, including updating Hawaii Immunization Registry System (HIR), Electronic Health Records (EHR), and other records; prepares immunization information; stores, administers immunizations per health plan and VFC guidelines.

Identifies patient immunization requirements according to age‑specific CDC guidelines; completes appropriate paperwork; facilitates vaccine health plan and VFC inventory control; orders vaccine and maintains inventories.

Uses file maintenance to enter newly arrived vaccines into the lot manager system; ensures compliance with health plan, federal and state guidelines; completes annual VFC enrollment training; maintains all records related to VFC program.

Trains new and existing LPNs on emergency vaccine storage procedures; serves as liaison with state to coordinate VFC inventory; generates reports and contacts patients for immunization compliance and updates.

Assists in ensuring compliance with infection control policies and protocols; follows medication administration, storage procedures, health plan, CDC, state and federal VFC program guidelines; provides educational materials; reviews printed instructions; collaborates to revise teaching plan.

Participates in orientation of new personnel; demonstrates sufficient knowledge and skills; performs clean/sterile technique, sets up for procedures, assists with clinical procedure within department scope; follows recommended process for handling point‑of‑use instruments per high‑level disinfection committee.

Maintains, orders, organizes, rotates, and stocks supplies; schedules patients for routine, acute care and care gaps; prepares and processes patient forms.

OHS only: Certification in Spirometry, Hearing Conservation, & Substance Abuse Specimen Collection or acquire certification within one year of hire.

Demonstrates knowledge, skills, and abilities necessary to provide care appropriate to age groups served.

Demonstrates culturally sensitive care.

Performs other duties and accepts responsibility as assigned within scope.

Education

Hawaii State Board of Nursing approved curriculum for LPN licensure.

License, Certification, Registration

Licensed Practical Nurse (Hawaii) required at hire.

Basic Life Support from American Heart Association.

Additional Requirements

Demonstrated knowledge of and skill in conflict resolution, customer service, interpersonal relations, oral communication, problem solving, teamwork, and written communication.

Talking to co‑workers, customers, outside vendors, and on the telephone.

Reading, writing, speaking and understanding English.

Training/giving and receiving instructions.

Mathematical ability, attention to detail, concentration and alertness.

Preferred Qualifications

Previous nursing experience.

Previous clinic experience.

Knowledge of word processing and spreadsheet PC applications.
