Social Worker (MSW)

Overview The Social Worker is responsible for providing high quality patient care, and demonstrates skills in assessment, treatment planning, group treatment, individual treatment, family treatment, and discharge planning. The role includes functioning as a member of the multidisciplinary team.
Responsibilities
Knowledge of social work programs and community services, and ability to coordinate linkages with community resources to ensure continuity of care at discharge.
Knowledge of the care and handling of residents of all age groups and understanding of the special needs and behavior of each age group.
Conduct thorough intake assessments and psychosocial histories, identify significant problems for the treatment and discharge plans, and document progress in medical records.
Provide emotional support to patients and families to cope with the stress of psychiatric hospitalization and separation from family and home.
Document significant contacts in progress notes consistently with designated timeframes.
Participate in daily patient reviews/rounds with team members.
Participate in weekly treatment plan meetings and complete the Comprehensive Treatment Plan and Treatment Plan reviews.
Ensure continuity of care at discharge, as documented on the Discharge Summary.
Coordinate linkages with community resources and service providers.
Understand the need for, and respect the cultural and socio‑economic diversity of our patient population.
Qualifications
Education:
Master’s Degree in Social Work from an accredited school, required.
Experience:
Minimum of one (1) year clinical experience in outpatient or inpatient social work.
Knowledge and Skills:
Ability to communicate effectively in English, both verbally and in writing. Bilingual is preferred but not required. Ability to hear, talk, walk, stand, bend, and stretch. Basic computer knowledge. Ability to read, write and interpret documents. Must be trained in and knowledgeable of all Kingsbrook/Rutland policies and procedures pertaining to hazardous waste and hazardous materials and maintain qualifications applicable to assigned work.
Licenses, Certifications:
New York State Social Worker License (LMSW/LCSW) required. Acceptable upon hire for one year: valid NYS Limited Permit (LP), or eligible and receives LP within 60 days of hire.
Physical Requirements:
Involves standing for long periods at the patient bedside, walking, sitting for long periods at a desk, talking, hearing, bending, and may include repetitive motions. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions without compromising patient care. May work in surroundings with dust, noise, etc. without undue discomfort.
