You’ll manage a caseload, develop care plans, and collaborate with an interdisciplinary team to ensure comfort and dignity for every patient.**Key Responsibilities:*** Complete initial and ongoing patient assessments to determine hospice needs* Provide professional nursing care using the nursing process* Develop individualized care plans incorporating palliative interventions* Administer medications and treatments as prescribed* Counsel patients and families on health needs and coping strategies* Educate patients and families on care instructions* Assist with activities of daily living and promote comfort care* Act as Case Manager for assigned patients, coordinating care across disciplines**Qualifications:*** Active California RN license* Minimum 1 year of nursing experience (**hospice experience preferred**)* Ability to lift, position, and transfer patients; physical stamina for prolonged standing and walking* Current CPR certification* Valid driver’s license and insured, reliable vehicle**About Seaport Hospice**Seaport Hospice is part of the **Pennant Group**, a nationwide network with over 300 locations.
