Position Highlights • Setting: Home Health • Schedule: Monday–Friday, daytime; flexible scheduling; weekends off; autonomy to stack visits • Location: San Francisco, CA (serving patients within a 5–10 mile territory radius) • Population Served: Adult and geriatric home-based patients • Compensation: $130,000–$190,000 per year (DOE) • Perks & Bonuses: – Paid parking and bridge tolls – Overtime eligible at per-visit per-diem rate – Medbridge access (CEUs + patient education tools) – Extensive documentation and admin support – Strong onboarding, training, and leadership growth pathways
Key Responsibilities • Evaluate patient needs and develop individualized home-based physical therapy plans. • Deliver evidence-based therapeutic interventions to restore mobility and reduce pain. • Track patient progress and adjust treatment plans to support functional improvement. • Educate patients and caregivers on exercises, safety, and mobility strategies. • Collaborate with an interdisciplinary care team to ensure quality outcomes. • Maintain accurate, timely documentation using user-friendly digital tools. • Promote patient independence and provide compassionate, supportive care.
Qualifications • Physical Therapist (PT) — active, unrestricted California PT license • Minimum of 1 year PT experience • Current AHA CPR certification • Valid Driver’s License and reliable transportation • Strong communication, time-management, and clinical reasoning skills
