About You We are seeking someone who will partner with the client development department, residences, and outpatient clinics. Responsibilities include facilitating tours, reaching out to hospitals and referents, coordinating and completing assessments. Collecting and understanding admission documentation as well as CCL charts. You will need: Full Vaccination against COVID-19 – CA required Bachelors degree (health care administration, nursing, public administration, psychology) Five years of professional behavioral health care experience LVN/LPT certification - required We compensate for your accomplishments and talents! Your Responsibilities Your role as a Lead Mental Health Admission Specialist you will, Assess potential clients through in-person interviews and documentation from existing placement, and visits potential clients regularly to review progress towards discharge to Psynergy facility. Follow up with case managers and conservators to determine readiness and timing of placement into Psynergy Programs. Complete all pre-admission forms, intake, and Community Care Licensing (CCL) documents as required by CCL. Follow up with the Counties to receive County assessments, treatment plans, and initial assessments for the benefit of Psynergy’s outpatient clinic client files. Gather and completes records and other required documentation in accordance with agency, payer and regulatory requirements. Understand and support company service policies and procedures to provide proper and effective service to all customers. Foster the development of long term business relationships. Submit appropriate paperwork to management such as expense reports and call reports in accordance with Psynergy policies. Attend and participates in staff program, agency and community meetings and training as required. Transport clients when appropriate. Have ideas or skills that would help our clients? Let us know!
