Licensed Practical Nurse

Summary The Licensed Practical Nurse provides direct patient care under the supervision of a Registered Nurse, Regional Nurse Manager or Medical Provider.
Responsibilities
Accurately collects clinical data on patients and demonstrates competency in performing clinical interventions and reports abnormalities to Medical Provider or RN
Documents nursing observations, interventions, and patient’s response to treatment in patient records
Demonstrated competency in performing venipuncture for collection of blood specimens and facilitates the prompt and accurate processing and distribution of specimens collected
Ensures that patients receive timely and appropriate care within the delivery system and facilitates internal and external referrals in collaboration with the RN
Attends internal and external educational seminars/ staff trainings
Performs all related duties, as assigned, or unrelated duties, as requested, during emergencies
Administer prescribed medication to clients and keeps accurate records of all medication administered in accordance with law and Sun River Health policies and procedures
Ensure that SRH standard procedures and workflows are implemented and followed, including but not limited to: medication management, pre‑visit planning, team huddles, accurate and timely completion of Nursing Quality Logs, exam room readiness, ordering and storing supplies and equipment
Demonstrate knowledge of the principles of growth and development over the life span, and provide age‑appropriate and culturally sensitive care, including use of medical interpreters as indicated
Functions as preceptor to students, new hire candidates and other clinical team members/ staff when needed
Utilizes standing orders in accordance with Sun River Health policies and procedures
Attend mandated training and demonstrate competence on all nursing procedures, including relevant lab procedures
Perform direct patient care duties of an LPN: interview patients for health history, prepare equipment and assist practitioner during examination and treatment of patients, document in patient’s medical record according to SRH standard and gather information from patients as needed.
Provides educational information to patients for health promotion and disease prevention to improve health outcomes
Requirements
Required: High school diploma/ GED/ Vocational/ Technical School/ Diploma Program – Nursing required
Minimum Work‑Related Experience: 0‑1 year
Preferred Work‑Related Experience: 2‑4 years
Minimum Licensure and Certifications Valid New York State LPN License
CPR/BLS‑HCP
