Responsibilities
Assess the patient’s physiologic health status
Conduct patient interview and explain policies
Review patient chart and respond to questions courteously
Assess gastrointestinal, cardiovascular, respiratory, renal and neurological health status
Determine mobility, sensory deficits, prostheses use and skin condition and adjust plan of care as appropriate
Assess level of pain and manage pain
Communicate and document patient’s physiologic health status and plan of care
Assess patient’s psychosocial health status
Elicit perception of care and expectations of care
Determine coping mechanism, knowledge level, and ability to comprehend
Identify cultural/ethnic requirements to reduce anxiety and ensure patient satisfaction
Communicate and document psychological status and care plan
Support group, counseling services, social services
Identify patient outcome
Develop criteria for measurement of patient outcomes
Identify actual/potential patient problems
Identify patient’s need for teaching based on psychosocial and developmental assessment
Develop patient outcome statement(s) and establish individualized patient goals
Formulate the plan of care and establish priorities necessary to achieve expected outcome
Identify care activities and establish priorities necessary to achieve expected outcome
Coordinate cost‑effective use of supplies, equipment and medication to achieve expected outcome
Document plan of care and collaborate with physicians and other health team members
Implement the plan of care
Maintain constant vigilance over patient care to ensure safety precautions are followed
Exercise professional skills related to the plan of care
Reassess patients as needed and appropriately revise plan of care
Correctly administer prescribed treatments
Use equipment necessary for patient care appropriately
Provide emotional support to patient and significant other
Apply scientific principles in performing procedures safely and efficiently
Make accurate observations of patient’s conditions during treatments/procedures and document accordingly
Keep accurate documentation of treatments, activity and condition
Use appropriate methods of documentation according to departmental policy
Act rapidly and effectively during any emergency
Provide a calm, quiet, restful atmosphere and communicate effectively with patient’s family or caretakers
Participate in planning for discharge and coordinate referrals as appropriate
Provide discharge instructions to patient and significant other
Evaluate care provided for patient outcome
Demonstrate ability to measure effectiveness of care provided and document results
Perform variance analysis related to outcome data for performance improvement
Design, implement and evaluate systems to improve care in the unit
Use appropriate methods of documentation according to departmental policy
Collaborate with other care team members and provide direction
Collaborate with appropriate physician on patient’s plan of care
Interpret and implement treatment regimen as prescribed by the physician
Assist physician during treatments and/or diagnostic procedures
Keep the patient care manager/physician informed of changes in patient condition or treatments
Delegate assignments to support staff and ensure timely completion
Perform grade I‑IV decubitus care
Preventive skin care measures
Apply simple dressings, maintaining aseptic technique
Apply warm & cold compresses, consistent with facility procedure
Perform irrigations consistent with facility procedure
Perform other procedures related to skin care as necessary
Administer medications correctly and safely
Correctly identify medication action, dosage, side effects and implications for the patient
Meet standard on medication administration examination
Prepare local solutions and IV drips
Administer and document medication correctly
Administer IVP medication per department policy and procedure
Educate patients and significant others about drug and food interactions
Participate in patient and family education
Provide the patient with an explanation of his/her condition as indicated
Communicate assessment data in an orderly fashion and update and verbalize pertinent information to care team members and agencies
Recognize and utilize health teaching opportunities and resources/materials available for teaching
Provide early discharge planning and appropriate referrals for post‑hospital care
Evaluate the effectiveness of teaching by feedback from patient/family and document results
Perform related duties as required
Qualifications
Graduate from an accredited School of Nursing. Associate’s Degree in Nursing, required. Bachelor’s Degree in Nursing, preferred.
Minimum 1-2 years of professional nursing experience in the related specialty.
Current license to practice as a Registered Professional Nurse in New York State and/or Connecticut, required.
BLS required, plus specialized certifications as needed.
