Job description: Job Title: Care Manager Job Type: Full-time

Position Summary: The role of the care manager is to ensure that DALS clients are properly assessed and properly cared for. To oversee staff, ensure they are trained and supported to provide exceptional and the highest quality of care to our clients

Essential Functions: Provide assessments to DALS clients and potential clients. Provide supervision to DALS staff to ensure that our clients come first and that our Deaconess caregivers have the support they need to be able to provide excellence in care that exceeds the Clients’ expectations. Collaborate with staff in delivering exceptional client service that surpass quality measures. Create and contribute to a warm and welcoming organization grounded in customer service. Report daily to the Executive Director of DALS any significant change in condition/status of Clients or Families. Attend and participate in Daily Stand up. Collaborate and support the Home Office team on clients, scheduling and other projects. Attend and participate in monthly QAPI meetings, meeting/exceeding established measures. Maintain Quality reports as requested by Executive Director. Attend and participate in Monthly Town Hall Meetings. Engage in DALS growth initiatives. Regular rounding at local hospitals, SNFs, ALs, ILs. Responds to requests for referral. Completes in person assessments as indicated by client needs. On call responsibility as required including weekend rotations with other care managers.

Client Care: Provide assessment at Start of Care and any significant change in condition. Document detailed care plans. Provide and document in person monthly Supervision visits of Aides and monthly phone call to Clients/Families. Collaborate with DALS team and review care delivery at daily standup. Collaborate with Executive Director on potential and current residents for services and care. Collaborate with DALC (Deaconess Abundant Life Communities) Colleagues in coordination of care for Clients. Collaborate with external care partners. Engage clients’ family members to support the overall well-being of their clients. Establish relationships with potential Clients/Families. Care plan in line with DALS care plan standards.

Management of Staff: Supervise/monitor staff in delivery of care and ensure staff have all the appropriate tools to deliver the highest quality of care to our clients. Orient each caregiver on the detailed care plan as they are assigned to new clients. Participates in and conducts interviews for potential new hire caregiver candidates. Present clinical orientation to new hire and ensure that orientation Assists in completing annual caregiver reviews in conjunction with HR. Ensure staff is following policies and procedures and delivering quality care. Follow reeducation and disciplinary guidelines as necessary in conjunction with Executive Director & Human Resources. Conduct monthly supervisory visit for each client and document in Customer Relationship Management Software. Prepare additional education for further identified educational needs.

Community Outreach/ Marketing: Establish and maintain relationships with our referral sources and care partners. Attend networking events and participate in outreach to the community. Participate in assigned scheduled marketing meetings/events.

Qualifications: Experienced in a clinical field (RN, LPN, PT, OT, SLP). Maintains a current Massachusetts clinical license in good standing. Strong assessment and organizational skills. Demonstrated success in working independently while taking on increased responsibilities. Solid communication and computer skills. Strong leadership/management qualities, including ability to work with different groups of staff, co-workers, and referral sources is required. Be able to multi-task in a fast-paced healthcare environment. Travel is required.
