About the Role:
As an APP in post-acute care, you will provide in-person care to patients in skilled nursing and rehabilitation facilities. You will play a critical role in managing both chronic and acute conditions, participating in transitional care planning, and supporting the facility?s quality improvement initiatives.
Core Responsibilities:
Collaborate with Trumen physicians and facility medical directors to manage patient care
Conduct comprehensive clinical assessments and manage treatment plans
Participate in transitional care planning and prevent avoidable hospital readmissions
Reconcile medications and provide patient education
Foster positive working relationships with facility leadership, clinical staff, and care coordinators
Participate in Trumen?s value-based care programs by tracking patient outcomes and supporting quality improvement initiatives
Serve as a liaison between Trumen and assigned skilled nursing facilities (SNFs)
Maintain accurate and timely documentation within the EMR
Be present on-site during regular business hours, depending on availability
Qualifications:
Active
Nurse Practitioner or Physician Assistant license
in the state of
Texas
Minimum of
1 year experience
as an APP, preferably in a post-acute, SNF, or geriatric care setting
Ability to work independently while effectively collaborating with the care team
Strong interpersonal skills and a passion for caring for complex patients
Reliable transportation to the facility
