Nurse Practitioner or Physician Assistant ? Telehealth Care (On-Site Only)

In-Office Telehealth with In-Home Medical Assistant Support
Compensation:
$80?$100 per hour
Schedule:
Sunday?Thursday, either 2 PM?9 PM or 3 PM?10 PM (on-site only)
A fast-growing, innovative healthcare provider in Crown Heights is seeking a
Nurse Practitioner (NP)
or
Physician Assistant (PA)
to lead in-office telehealth visits for a diverse patient population. In this unique model, the provider works from the office while a trained Medical Assistant visits the patient?s home to assist with exams, vitals, and procedures.
Key Responsibilities: Conduct telehealth visits for patients of all ages, including family medicine, pediatrics, and geriatrics
Evaluate and treat common conditions such as strep throat, flu, RSV, rashes, eye infections, and seasonal allergies
Diagnose and manage ENT-related issues such as ear infections and sinus symptoms
Coordinate with on-site Medical Assistants to assist with specimen collection (e.g., strep cultures, flu and COVID-19 testing), basic physical exams, and vitals in the patient?s home
Ensure accurate charting and documentation in the EMR system
Deliver compassionate, accessible, and patient-centered care
Requirements: Licensed NP or PA in New York State
Clinical experience in family medicine, urgent care, or primary care settings
Comfortable working with telehealth platforms and in-office telehealth workflows (this is not a remote position)
Strong clinical judgment and ability to work independently while collaborating with a team
Excellent communication and organizational skills
