Actify Home Care . Actify Home Care is looking for a Speech‑Language Pathologist (SLP) to join our growing team! We have an immediate need to assist with growing geriatric patient care needs throughout Broward County. Responsibilities
Accept and schedule patients for home visits typically lasting 45 minutes to 1 hour. Assess the skilled Speech Therapy needs of typically geriatric, homebound patients who are often being treated for cognitive and dysphagia, dysarthria, apraxia, and aphasia diagnosis. Develop a care plan to be carried out by yourself in coordination with the approved care plan. Document the condition and plan for each patient via electronic visit notes. Practice infection control best practices to ensure your safety and patient safety. Communicate patient needs with our care team to ensure all necessary services are provided. Operate as an independent contractor, signing and submitting notes on a single EMR system in real time. Contact us now to get started. Do you have home health experience? This is a great opportunity to supplement your current caseload or transition to a more independent system. Even if you don’t have home health experience, you can learn how to translate your outpatient, SNF, or rehab experience into a home‑health setting. You will operate as an independent clinician and be paid $75 per session, with ongoing supportive consulting and resources from our team. Compensation & Benefits
