Responsibilities
Provide care and services for a group of patients, including home visits, skilled nursing facilities, and assisted living facilities.
Work as a contributing member of the interdisciplinary team, offering expertise in psychosocial assessments and community resources.
Serve in either a Case Management, Admissions, or Resource role.
Collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership.
Work independently with day‑to‑day autonomy, managing phone, email, and written communication, and attending meetings with various healthcare professionals.
Manage visits, documentation, and meetings, prioritizing and triaging multiple concerns.
Learn and use an electronic medical records system.
Be willing to drive with reliable transportation, a valid driver’s license, and auto insurance to travel across the service area.
Qualifications
Current Registered Nurse (RN) licensure in good standing in the state(s) of practice.
1 year of hospice experience.
Strong teamwork skills and ability to collaborate daily with a multidisciplinary team.
Excellent time management and organizational skills.
Ability to learn an electronic medical records system.
Valid driver’s license and reliable transportation for travel across the service area.
