About the Role:
We are seeking an experienced Nurse Practitioner with strong clinical expertise in diabetes management to join our healthcare team on a flexible, part-time basis. Responsibilities:
Provide patient education on diabetes management, including nutrition, glucose monitoring, and medication adherence.
Support patients enrolled in Chronic Care Management (CCM), Remote Patient Monitoring (RPM), and weight management programs.
Coordinate care and develop management plans for patients with complex diabetic conditions.
Monitor patient progress, adjust care plans as needed, and maintain accurate clinical documentation.
Communicate effectively with patients and multidisciplinary teams to achieve optimal outcomes.
Qualifications:
Licensed Nurse Practitioner (NP) in good standing.
Strong clinical experience in diabetes management or endocrinology (2+ years preferred).
Excellent patient education and communication skills.
Ability to work independently in a part-time, flexible schedule (20-24 hours per week).
