Territory: Marin County What you’ll do as a Registered Nurse (RN) Home Health Case Manager:
Utilize your clinical skills to create, implement, and execute comprehensive plans of care that promote wellness and prevent readmission.
Teach patients and family/caregivers wound care, blood draws, tube feedings, tracheostomy care, Foley catheters, CHF management, and medication education.
Perform a full assessment of the patient and family during the initial visit.
Use assessment data to develop an advanced plan of care, including rehabilitation.
Work one‑on‑one with patients and families to achieve improvement and/or stability.
Case‑manage the patient’s care episode with an interdisciplinary clinical team.
Prepare clinical notes, orders, and charting within a compliant timeframe.
Collaborate with physicians and agency staff to deliver the best care possible.
Initiate and administer IV medications (antibiotics and diuretics) safely and effectively per protocol.

To be successful as an RN Home Health Case Manager:
Current and unrestricted CA RN license.
Current BLS/CPR certification.
Valid CA driver’s license.
High level of clinical knowledge in home health care (highly preferred).
Experience with high‑acuity patients (plus).
Experience working with an interdisciplinary team (highly desired).
Fundamentals of case management (highly desired).
Previous experience with IV medication infusions (highly preferred).
Proficiency initiating intravenous therapy (plus).
Self‑directed, empathetic, critical thinker.
Outstanding interpersonal and leadership skills; organized.
Completed computer literacy and competency assessment prior to interview.

Wage Range Registered Nurse, Level 1: $130,000.00 – $160,000.00 plus $10,000 annual bonus, paid quarterly. Starting pay is commensurate with relevant experience above the minimum requirements. 21st Century offers generous bonus incentive plans, comprehensive health benefits and 401(k) (up to 4% match).
