Physical Therapist Case Manager: Home Health

Base pay range:
$120,000.00/yr – $135,000.00/yr. Tiered bonus program can increase compensation by up to $10,000 annually.
Full‑time, on‑site field role visiting patients in their homes.
Key Responsibilities
Perform home visits to evaluate needs and develop individualized care plans.
Conduct physical therapy sessions, supervise PTAs, and monitor patient progress.
Coordinate with physicians, nurses, and allied health staff to ensure quality, continuity of care.
Document assessments, treatment sessions, and progress reports in the electronic medical record.
Provide patient and family education on self‑management, home modifications, and safety.
Assist with administrative tasks and documentation to maintain billing accuracy.

Qualifications
Current unrestricted California Physical Therapist license.
BLS/CPR certification.
Valid California driver’s license.

Knowledge of local community resources and state/federal home‑care regulations.
Strong case‑management fundamentals and interdisciplinary teamwork experience.
Self‑directed, empathetic, critical thinker with excellent interpersonal and leadership skills.
