Job Description Summary The GIP RN admits, plans, organizes, and directs hospice care for General Inpatient (GIP) clientelle at our contracted hospitals. The role requires experience in nursing with an emphasis on community health education/experience. The professional nurse builds from community resources to plan and direct services to meet the needs of individual and families while admitted at the hospital.
Schedule: Three (3) ten (10)-hour shifts Tuesday - Thursday.
Essential Job Functions / Responsibilities Patient Care
Completes a comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illnesses.
Provides professional nursing care utilizing all elements of the nursing process.
Writes and initiates plan of care, regularly re‑evaluates patient and family/caregiver needs, and revises plan of care as necessary.
Uses health assessment data to determine nursing diagnosis and develops a care plan that establishes goals based on nursing diagnosis and incorporates palliative nursing actions, including patient and family in the planning process.
Initiates appropriate preventive and rehabilitative nursing procedures and administers medications and treatments as prescribed.
Counsels the patient and family in meeting nursing and related needs.
Provides health care instructions to the patient as appropriate per assessment and plan.
Assists the patient with activities of daily living and facilitates self‑sufficiency and comfort care.
Acts as liaison, obtaining required consents, necessary documentation for medical records, and coordinating with hospital staff.
Communication
Completes and submits accurate clinical notes regarding patient condition and care.
Communicates with physician regarding patient needs and reports changes; obtains and receives physician orders as required.
Coordinates with community health providers to implement the care plan.
Reports failures or concerns to appropriate authorities per state and federal law.
Teaches self‑care techniques to patient/family; provides medication, diet, and instructions as ordered.
Maintains a safe environment for the patient.
Assists in continuity of care and attends interdisciplinary group meetings.
Additional Duties
Participates in on‑call duties as defined by the policy.
Ensures arrangements for equipment and other necessary items and services are available.
Supervises ancillary personnel and delegates responsibilities if required.
Maintains professional knowledge through continuing education and in‑service classes.
Fulfills optional case assignments and participates in quality assessment and performance improvement teams.
Position Qualifications
Graduate of an accredited school of nursing.
Registered Nurse with a license in good standing to practice in Washington.
Minimum one (1) year of professional nursing experience within the last three (3) years; or a baccalaureate degree in nursing and at least two (2) years of experience, at least one of which is in public health, home care, or hospice nursing (preferred).
Experience supervising hospice aides is a plus.
Appropriate references and criminal background checks are required after offer.
Excellent observation, verbal and written communication skills, problem‑solving skills, basic math skills, and nursing skills per competency checklist.
Physical ability to lift, raise, and transfer patients; ability to perform prolonged walking, standing, bending, kneeling, and crouching as required.
Licensed driver with a valid automobile insurance that meets state and organizational requirements.
