- Yearly case volume: 500 per FTE
- Types of Cases - General (Community Hospital), including:
- OB, Spine, Ortho, Gen, GI, Hearts (if TEE certified), pediatrics
- Site does OB but not a requirement
- EMR
- Epic

- Must be board certified
- Must have active IMLC with LOQ in hand, or IN license
