Role Description This part-time position as a Clinician (NP, MD, or PA) focuses on providing home-based geriatric care in the San Francisco Bay Area. Primary responsibilities include performing comprehensive medical evaluations, managing chronic conditions, and collaborating with a multidisciplinary care team to deliver personalized care plans for older adult patients. The Clinician will leverage Sabio Health’s tools and resources to deliver coordinated, high-quality, patient-centered care. This is an on-site role requiring travel to patients’ homes.
Qualifications
Active and unrestricted licensure as a Nurse Practitioner, Physician, or Physician Assistant. Board certification in a relevant field is strongly preferred.
Proficiency in conducting health assessments, managing chronic diseases, and delivering high-quality patient care.
Strong interpersonal skills and the ability to build trusting relationships with patients and their families.
Experience with value-based care, palliative care, or home-based healthcare is a plus.
Effective communication, teamwork, and ability to operate within a multidisciplinary team.
Willingness and ability to travel within the San Francisco Bay Area to deliver in-home care services.
