The Medical Case Manager will work as part of a specialized team within the Primary Care setting of Sun River Health to provide medical case management services to HIV+ patients enrolled in the Ryan White Program. Services include, but are not limited to, assessing patient needs and barriers to care; service plan development; care coordination; referrals and linkages; education and support by sharing effective strategies to manage their health. The Medical Case Manager will utilize motivational interviewing and a harm reduction approach to provide trauma‑informed, supportive interventions. In addition, the Medical Case Manager will provide assistance to individuals not enrolled in case management but in need of situational support.

Pay:

$23.00–$25.24

Essential Duties

Provide telephonic care coordination services to Medicare‑eligible patients.

Coordinate medication refills, referrals, and diagnostic imaging results.

Document all services in eCW and communicate with internal clinical or behavioral health teams.

Assist in linking patients to services for daily activities, food insecurity, and home‑care assistance.

Minimum Experience & Education Requirements

High School Diploma or Equivalency (proof required).

Associate’s/Bachelor’s Degree (preferred).

Working knowledge of Medicare policies.

Experience with linkages to care and working with underserved communities.
