Position Overview We are seeking a
licensed Physical Therapist
to provide in-home physical therapy services to patients throughout Falls Church, Alexandria, Arlington, and surrounding areas. This role offers autonomy with strong clinical and administrative support.
Responsibilities
Perform in-home physical therapy evaluations and reassessments
Develop and implement individualized treatment plans
Educate patients and caregivers on treatment plans and safety
Document care accurately and timely in EMR
Collaborate with an interdisciplinary care team
Qualifications
DPT or MPT
from an accredited Physical Therapy program
Active Virginia Physical Therapy license
CPR/BLS Certification
Valid driver’s license and reliable transportation
Strong communication and time-management skills
Home health experience preferred, but not required
