The Nurse Anesthetist is an advanced practice nurse (CRNA). Under the supervision of an Attending Anesthesiologist, the CRNA administers, monitors and manages patient care before, during and after anesthesia.
Responsibilities
Clinical Practice Designated Attending Anesthesiologists/ Vice President, Nursing and Chief Certified Registered Nurse Anesthetist for performance appraisals and scheduling The performance of pre-anesthesia assessment, induction and maintenance of and emergence from anesthesia, monitoring of the patient\'s physiological condition and performance of clinical intervention as necessary, under the supervision of an Attending Anesthesiologist. The ongoing pre-anesthesia assessment in conjunction with Attending Anesthesiologist; obtain and review relevant tests and discuss findings with the Attending. Obtain and/or verify informed consent for the planned anesthetic intervention from the patient or legal guardian; discuss options and risks with the patient and/or guardian after discussion with Attending Anesthesiologist; document in the patient\'s medical record that informed consent was obtained. Formulate a patient-specific plan for anesthesia care in conjunction with the Attending Anesthesiologist, based on comprehensive patient assessment, problem analysis, procedure type, patient/surgeon preferences, and current anesthesia principles. Induce anesthesia under the supervision of an Attending Anesthesiologist and maintain anesthesia at required levels; continuously assess the patient\'s response and notify the Attending of significant changes; intervene as required to maintain physiologic stability. Monitor the patient\'s physiologic condition appropriate for the anesthesia type and patient needs. Monitor ventilation continuously; verify tracheal intubation; monitor end-tidal CO2; use spirometry and ventilatory pressure monitors. Monitor oxygenation by clinical observation, pulse oximetry, and arterial blood gas analysis when indicated. Monitor cardiovascular status via ECG and heart sounds; record blood pressure and heart rate; monitor temperature (especially pediatric/general anesthesia) and neuromuscular function when neuromuscular blocking agents are used; assess patient positioning and protective measures. Document omissions of any monitoring standards with rationale in the anesthesia record; remain in constant attendance until another qualified provider assumes responsibility. Maintain complete, accurate, and timely documentation of pertinent information in the patient\'s medical record; document all anesthetic interventions and patient responses. Transfer responsibility for patient care to other qualified providers with continuity and safety in mind. Adhere to safety precautions to minimize risk; document that the anesthesia machine and equipment were checked. Inspect the anesthesia machine prior to use and monitor according to guidelines; ensure equipment readiness, cleanliness and proper working condition; monitor breathing system integrity when using automatic ventilators; monitor oxygen concentration with an alarm system. Treat with precautions to minimize infection risk to patient and staff. Professional Practice Participate in ongoing review and evaluation of anesthesia care quality and appropriateness as a department member; evaluation based on outcome criteria and ongoing review. Engage in self-evaluation and incorporate new techniques and knowledge into practice. Participate in departmental Grand Rounds, presenting relevant cases as appropriate. Respect and maintain the basic rights of patients. Assist in research projects as directed by an anesthesiologist. Participate in instruction and supervision of Student Registered Nurse Anesthetists (SRNAs). Administrative Practice Collaborate with health care team to develop, implement and review policies, procedures and standards of care for a designated patient population. Evaluate and document patient care outcomes for the Department of Anesthesia and influence staffing and standards of care decisions. Identify performance improvement indicators and develop quality improvement programs with the team. Participate in risk management activities and communicate risk in a timely manner. Respond to budgetary constraints and incorporate financial principles into program development and patient care activities. Identify trends and potential problems in perioperative care and implement action plans and educational programs for faculty and staff. Qualifications
