Primary Duties and Responsibilities
Under the direction of a registered nurse or physician and under assigned administrative supervision, provide care within the scope of the Vocational Nurse Practice Act and regulatory standards.
Assist in the assessment, planning, implementation and evaluation of patient care.
Follow patient identification procedures, provide appropriate information for all patient procedures, and collect and handle specimens.
Assess and implement prescribed interventions for the patient.
Implement and document discharge plan for each patient.
Recognize that ethical/legal/medical dilemmas exist in the nursing practice and confer with appropriate resource personnel when making ethical nursing decisions.
Participate in maintaining a safe patient care environment.
Assist with the care and maintenance of department equipment.
Complete and accurate charting of patient records and record keeping of equipment and quality measures.
Collaborate with other disciplines of the health care team to ensure the delivery of care across the continuum, providing timely, safe, and quality care.
Responsible for providing a safe, HIPAA compliant environment for the patient and staff.
Perform other duties as assigned.
Home Health / Hospice Float Provide care in both the home health and hospice setting. Assignments will be throughout Orange County, patient care territories, and may change daily.
Qualifications Education and Experience
High School diploma or equivalent.
One or more years of experience in a healthcare care facility (Specialty Clinics can hire a new grad LVN with no experience).
Basic keyboarding skills and knowledge of Microsoft Office Suite, including Outlook, Word and Excel preferred.
Basic knowledge of medical terminology preferred.
Experience with Electronic Medical Records preferred.
Hospice: Community health/hospice or medical/surgical experience preferred. Understands philosophy of hospice concept and needs of the terminally ill. Must be a licensed driver with an insured automobile in good working order.
License Required Current LVN License to practice in the State of California.
Certifications Required Current BLS Certification.
Certifications Preferred Phlebotomy Certification.
