Registered Nurse – Outpatient Infusions Boulder Community Health, Boulder, CO 80303

$39.41 - $59.12 an hour -

Per Diem Days, Various shifts and hours per week.

*Range includes per diem differential. +$2.50/hr for weekend shifts.

Boulder Community Health is seeking aRegistered Nurse for the Outpatient Infusion department. Boulder Community Hospital (BCH) Primary Care Physician Clinics are guided by the philosophy of the Patient-Centered Medical Home. The medical home organizes care around patients, while engaging a multidisciplinary Care Team working proactively to coordinate and track care for each individual patient.

Qualifications

Colorado RN license in good standing.

BLS certification required by completion of conditional period.

Six months to one-year RN clinical nursing experience in medical/surgical nursing preferred.

Responsibilities

Provides direct total patient care for specific group of patients with collaboration as needed.

Demonstrates clinical competence in performing nursing procedures and identifies the rationale underlying the nursing skills.

Identifies patient care problems, makes interventions based upon an understanding of the bio-psychosocial processes.

Assesses patient status systematically and anticipates and responds to changes in patient clinical condition.

Actively seeks participation of the patient, family, and other members of the health care team to assure continuity of patient care.

Communicates clearly in all aspects of documentation. It is the expectation that all care providers document accurately and in real time, utilizing the electronic medical record within their department.

Practices universal precautions appropriately. Adheres to Standard Precautions as appropriate.

Performs point of care laboratory testing.

May be required to do moderately complex lab testing.

Collaborates with interdisciplinary patient care team to identify patient learning needs, develops and implements an individualized teaching plan.

Management and Decision Making

Utilizes effective time management skills in planning and providing patient care.
