Incentive Bonus up to 17.5%

Position Overview The Director of Case Management is responsible for the overall leadership, management, and coordination of hospital-wide case management operations. This role ensures compliance with organizational policies, regulatory requirements, and best practices in care coordination and discharge planning. The Director collaborates with interdisciplinary teams and external partners to promote continuity of care, optimize clinical resource utilization, and achieve key performance outcomes.
This position requires strong executive presence, advanced leadership skills, and deep expertise in acute care hospital case management. The Director serves as a subject matter expert and leads initiatives focused on quality, efficiency, length of stay reduction, and patient flow improvement.
Key Responsibilities
Direct and evaluate case management department operations, including staffing models, productivity, onboarding, and staff competencies
Allocate resources to meet operational, quality, and performance goals
Provide oversight of core case management functions and professional practice standards
Ensure timely, high-quality care coordination and discharge planning with appropriate documentation
Collaborate with finance and executive leadership to achieve performance and budgetary goals
Analyze operational and clinical data to identify opportunities for improvement and implement sustainable process enhancements
Ensure compliance with internal policies, procedures, and external regulatory standards
Establish and maintain effective working relationships with physicians, nursing leadership, ancillary services, compliance, legal, and other key stakeholders
Lead interdisciplinary teams to support patient throughput, capacity management, and continuity of care
Perform additional duties as assigned
Operational Scope
Oversees case management services across the entire hospital
Leads a multidisciplinary team of case managers and support staff
Supports a high-volume acute care environment, including trauma services
Administrative hours with occasional on-call or extended hours as needed
Qualifications
Bachelor’s degree required
Master’s degree in Nursing, Health Administration, or Business Administration preferred
Current licensure as one of the following required:
Registered Nurse (RN)
Licensed Clinical Social Worker (LCSW)
Licensed Master Social Worker (LMSW)
Minimum of 3 years of recent acute care hospital case management experience
Minimum of 2 years of leadership experience in case management
Demonstrated success in reducing length of stay, managing capacity, and driving performance improvement
Strong data analysis, process improvement, and change management skills
Proven ability to lead teams, influence stakeholders, and manage complex initiatives

Pay Details:
$109,762.00 to $135,000.00 per year
