Clinical Pharmacy Specialist

Summary This position is eligible for the Education Debt Reduction Program (EDRP) – a student‑loan reimbursement program.

Eligible candidates must meet specific criteria per VHA policy and submit their EDRP application within four months of appointment.

Award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the application.

Former EDRP participants are ineligible to apply.

Qualifications Applicants who have not yet completed educational or certification/licensure requirements may be referred and tentatively selected, but may not be hired until all requirements are met.

Basic Requirements

United States Citizenship: Non‑citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA policy.

Education: Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree.

Foreign graduates must provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification – which includes passing the FPGEE and the TOEFL iBT.

Licensure: Full, current, and unrestricted license to practice pharmacy in a State, Territory, Commonwealth, or the District of Columbia.

The pharmacist must maintain current registration if required for full licensure.

Any license that has been revoked, suspended, denied, restricted, limited, or placed in a probationary status may be appointed only in accordance with VA Handbook 5005 – Part II – Chapter 3 – section B – paragraph 16.

May qualify under the Grandfathering Provision described in the VA Qualification Standard for this occupation (applicable only to current VHA employees who are in this occupation and meet the criteria).

Grade Determination: In addition to the basic requirements, candidates must meet the following criteria when determining the grade of candidates:

Clinical Pharmacy Specialist – GS‑13 experience.

Must have 1 year of experience equivalent to the next lower grade level (GS‑12 requirements).

Duties

Clinical – Direct Patient Care Activities that require a scope of practice, including evidence‑based pharmaceutical care services through direct and indirect interaction with patients and providers.

Practice sites may include but are not limited to: Anticoagulation Management Clinics, Primary Care/PACT Clinics, Specialty Care Clinics, Mental Health Clinics.

Perform, review, and analyze appropriate laboratory tests and diagnostic studies to monitor and support patients’ drug therapy.

Follow up with patients on lab or test results and discuss therapy plans, including changes, monitoring, and additional testing.

Determine when a referral to a physician is necessary for disease progression or adverse drug events outside the CPS scope of practice.

Clinical – Indirect Patient Care Activities, including population management analyses, medication‑therapy reviews for clinical effectiveness, dose adjustments, and communication of findings to prescribers.

Document findings, monitor drug errors, adverse reactions, allergies, and compliance issues, and review requests for non‑formulary and restricted drugs for appropriateness.

Assist Pharmacy and Therapeutics committees with medication use evaluations and safety initiatives.

Promote compliance with drug‑therapy policies established by the facility and national VISN initiatives.

Education and Research: Participate in the training and in‑servicing of pharmacists, technicians, students, residents, and other staff.

Serve as preceptor or mentor for pharmacy students and residents, assist in orientation and competency assessment, and complete required training management system entries.

Drug Information: Provide authoritative drug interaction, dosage, and compliance information to medical, nursing, technician, and student staff.

Maintain up‑to‑date knowledge of therapeutics and disease management.

Program Management: Coordinate and supervise all pharmacotherapy management activities for assigned patients in the CPS clinic(s).

Schedule consults, provide patient care, ensure coverage of clinical services, and serve as an active member of assigned committees.

Information Security: Maintain system security and confidentiality, limit employee access to sensitive information, and report violations of information security.

Work Schedule: 8:00 AM – 4:30 PM, Monday – Friday.

Telework: Not available; this is not a virtual position.
