Role Description In partnership with the Case Manager, you will provide skilled services to assist patients with breathing, speaking, and swallowing functions; evaluate and treat speech, language, cognitive, and fluency disorders that affect communication; and serve as an educator, patient advocate, and member of the interdisciplinary care team.

Qualifications
Current unrestricted CA Speech Therapist License
Current BLS/CPR Certification
Valid CA Driver's License
Knowledge and understanding of individual development and human behavior as it relates to the effects of illness/injury, and of the influence of culture on health care
Knowledge of state and federal home‑care regulatory guidelines preferred
Experience in home‑health care highly preferred
Experience with high‑acuity patients a plus
Experience working with an interdisciplinary team highly desired
Case Management fundamentals highly desired

Prior to an in‑person interview, all candidates must complete a computer literacy and competency assessment to ensure success in electronic charting, shared understanding of company values, and use of our web‑based communication application

Anticipated wage range: Speech Therapist: $110,000.00 – $125,000.00 per year. Starting pay is commensurate with relevant experience above the minimum requirements. 21st Century offers generous bonus incentive plans, comprehensive health benefits, and 401(k) (Up to 4% match).
