Fairview Health Services is seeking a Physician Assistant to join our established primary care practice at M Health Fairview Clinics – Oakdale in Oakdale, Minnesota. This is an excellent opportunity to join a collaborative, multi-specialty clinic. Practice details
Established primary care clinic with 12 provider primary care team, including 9 Family Medicine Physicians and 2 Nurse Practitioners. Average patient volume of approximately 16–24 patients per day. Opening: 1.0 FTE (34 patient contact hours) – flexible schedule. Fully integrated electronic medical record (Epic). Responsibilities
The Physician Assistant is a member of the health care team who works with the supervision of licensed physicians to provide comprehensive health care services to patients. The Physician Assistant engages in independent decision making about health care needs and provides health care services to individuals throughout their life span. The Physician Assistant provides physical and psychological health assessments, patient education and counseling, orders and interprets diagnostic procedures, and manages chronic disease conditions ensuring continuity of care for individuals in the communities served. Physician Assistants also perform minor surgical procedures within their scope and in accordance with their practice agreement with their supervising physician. Systematically assesses patients’ health status, arrives at appropriate diagnoses, and implements care plans for effective clinical outcomes. Performs appropriate medical care within scope of practice as defined by the practice agreement with the supervising physician. Considers a thorough range of treatment and preventive options and recommends age-appropriate, culturally appropriate, realistic, and attainable courses of action. Manages patient care and health status following initiation of treatment/preventive plans, and monitors and adjusts as warranted. Communicates effectively with patients and families about diagnosis, treatment plans, and follow-up care. Educates and encourages patients to participate in their own health care and provides education materials to support wellness. Seeks consultation and communicates requests to supervising physicians in a courteous, professional, and timely manner. Documents according to established guidelines to facilitate ongoing quality of care. Delegates direct and indirect patient care activities to appropriate personnel. Responds appropriately to emergent situations. Maintains and improves skill level through continuing medical education and participates in clinical quality improvement initiatives. Reviews and follows up on complaints and concerns about medical care and takes actions to assure continuity of care. Contributes to efficient practice management policies to support patient access to care. Participates in access planning for the clinic site and ensures appropriate same-day patient access. Meets clinic patient contact hour requirements and budgets relative value units to achieve targeted goals. Fulfills call assignments with clear communication and follows through on work initiated. Supports appropriate billing for professional services through timely documentation and knowledge of billing policies and procedures. Adheres to relevant policies, procedures, and practice guidelines. Assists with preparation for JCAHO and other regulatory surveys. Responds to telephone calls in a timely fashion and manages patient care with appropriate use of resources. Prescribes and orders ancillary services appropriately and utilizes the Fairview referral network. Promotes appropriate length of stay and manages chronic health/disease conditions. Provides leadership for clinic and community relations and serves as a resource to colleagues and support staff. Participates in training, orientation, and evaluation of clinic support staff; attends and participates in clinic staff meetings; participates in appropriate committee structures. Participates in community education and other professional activities to promote Fairview Health Services and the site. Graduate of an accredited Physician Assistant training program. BC/BE through NCCPA. Licensed or ability to obtain a PA license with prescriptive authority through the Minnesota Board of Medical Practice. Ability to obtain and maintain DEA certification in the state of Minnesota.
