Base pay range $90,000.00/yr - $135,000.00/yr
Job Summary The primary purpose is to assist the DON in planning, organizing, and directing the day-to-day functions of the Nursing department to ensure that the highest degree of quality care is maintained at all times, as directed by the Administrator or the Director of Nursing. Reports to DON.
Responsibilities
Act as DON in absence of DON.
Assist the DON in planning, developing, organizing, implementing the day-to-day functions of the nursing department in accordance with all governing rules.
Assist in nursing education.
Provide in-service training for CNAs on a monthly basis, covering:
Catheter Care
Pericare
Infection Control
Oral care & hygiene
Oxygen Application
Safe transfers
Return demonstrations for CNAs.
Provide in-service training for Nurses on a quarterly basis, covering:
Infection Control
Oral Care and hygiene
Oxygen Administration
Medication Pass and proper documentation
Mantoux Testing
General nursing issues
Return Demonstrations for Nurses
Mantoux testing- semi annually
Medication Pass- semi annually
Participate in developing, maintaining, and updating written policies and procedures.
Ensure reference material at nursing station is current.
Make written and oral reports/recommendations to the DON regarding department operation.
Ensure that all nursing personnel follow their respective job descriptions.
Participate in QA program implementation for the nursing department.
Assist resident and Social Services in planning nursing portion of discharge plan.
Conduct chart audits:
Admission audit within 72 hours—follow 72‑hour protocol.
Audit all charts on each floor quarterly.
Analyze lab reports for floor, including:
Physician notification
Nurse noted lab and new orders.
Follow new orders through.
Manage PT/INR reporting and ensure proper actions.
Other documentation monitoring: Infection/Isolation, change of condition, review nursing notes for informativeness.
Handle incidents and accidents: investigations, policy updates, notify DON of reportable incidents before state reporting.
Administrative duties: complete medical forms, reports, evaluations, charting.
Serve on committees (Infection Control, QA, Safety). Attend Care Plan meetings.
Participate in weekend administrative duty and supervisory on‑call schedules.
Determine and maintain staffing needs in nursing department.
Assist in recruitment and selection of nursing personnel; ensure sufficient nurses and CNAs per shift.
Oversee scheduling work assignments and duty hours with co‑ADON and Staffing coordinator.
Assist in preparing performance evaluations.
Make daily rounds at least twice per day on floor to check:
Proper catheter bag placement
Residents groomed well
Oral hygiene
Proper positioning
CNAs using gait belts
Staff wearing name badge
Staff interacting with residents, families, and other staff appropriately
Oxygen tubing off floors
Humidifiers dated
Tube feedings documented with date, rate, product, time
Proper footwear Call lights within reach Monitor nursing care fairness, kindness, dignity, respect
Ensure privacy and knock before entering resident’s room
Signs of change in condition
Dehydration
Lethargy
Mental status change
Overall status
Monitor staffing, check call offs, ensure needs met, daily problem solving with staffing coordinator, participate in rotation to work floor as needed.
Dining Room: monitor meal service daily; ensure staff assigned to dining room timely; while feeding, staff should be seated.
Medication Pass: completed timely, carts locked, out of date meds, dated insulin, dated apple sauce, narcotics signed out, QA for glucometers, report errors to DON.
Monitor absenteeism, tardiness, overtime, creatively resolve issues.
Admin disciplinary action administered fairly.
Participate in selection of residents for admission.
Coordinate with Admissions Director for special needs (oxygen, special mattresses).
Greet new admissions upon arrival.
Make rounds with doctors when necessary.
Ensure residents unable to call for help checked frequently.
Attend continuing education programs to keep abreast of changes and maintain current license status.
Monitor nursing personnel safety regulations for equipment and supplies.
Maintain clean and safe nursing work areas.
Ensure personnel follow departmental policies, appropriate dress code.
Maintain adequate stock levels of medications, equipment, etc. on site.
Maintain confidentiality of resident care information.
Coordinate resident program.
