Home Health Primary Care Opportunity in Chattanooga, Tennessee
Lead a collaborative care team and deliver personalized medicine directly to patients in their homes. This Physician Pod Leader role offers a rare blend of clinical autonomy, leadership, and a sustainable Monday-Friday schedule, all within a mission-focused, tech-enabled organization. Make a meaningful difference in the lives of Chattanooga's most vulnerable patients while guiding a tight-knit care team.
Practice Overview
You’ll lead a Chattanooga-based team of 2 APPs and manage a personal panel of ~140 patients
100% home-based care with no clinic or hospital responsibilities
Blend of chronic disease management and coordinated team care in patients' homes
Serve as clinical lead: mentoring, chart review, oversight, and quality improvement
Schedule
Monday-Friday only; no weekends or after-hours requirements
Half of the week is dedicated to patient visits; the other half to leadership/admin duties
6-8 home visits per day, each lasting 30-40 minutes
Flexible, mobile schedule with care team and diagnostic support
Compensation & Benefits
Highly competitive base salary with guaranteed quality bonus in the first year

Candidate Requirements
Board-certified or board-eligible in Family Medicine, Internal Medicine, or Geriatrics
Leadership experience or interest in mentoring APPs and leading care teams
Strong interest in value-based care and home-based medicine
Must be licensed or eligible for Tennessee licensure
