Title: Psychiatric Nurse Practitioner – ACT Team

Location: Westchester County, NY

Responsibilities of the Psychiatric Nurse Practitioner

Provide psychiatric assessments, medication management, and ongoing clinical oversight to adults enrolled in the ACT program.

Deliver community-based care, meeting clients in their homes or other community settings as needed.

Collaborate with a multidisciplinary team including psychiatry, social work, nursing, vocational specialists, and peer support providers.

Participate in daily team meetings and treatment-planning sessions to ensure coordinated, person-centered care.

Maintain accurate and timely clinical documentation in accordance with regulatory standards.

Contribute to crisis intervention, risk assessment, and development of safety plans when necessary.

Support recovery-oriented, culturally responsive, and trauma-informed approaches to care.

Requirements of the Psychiatric Nurse Practitioner

Active New York State Nurse Practitioner License (Psychiatric–Mental Health Nurse Practitioner certification required).

Master’s or Doctoral degree from an accredited Nurse Practitioner program.

DEA registration or eligibility to obtain.

Experience working with individuals with serious mental illness; ACT Team or community-based psychiatric experience preferred.

Valid driver’s license and ability to travel within Westchester County for community visits.

Strong communication, organizational, and interdisciplinary-collaboration skills.
