About CAMBA CAMBA is a community of staff, volunteers, clients, donors, neighbors and partners who work together to build an inclusive New York City, where all children and adults have access to the resources and supports they need to thrive. We take a comprehensive approach by offering more than 180 integrated programs in Education & Youth Development, Family Support, Job Training & Employment Support Services, Health, Housing, and Legal Services. We reach almost 80,000 individuals and families, including almost 13,000 youth. CAMBA serves a diverse cross section of New Yorkers from new mothers in Brownsville to job seekers in the Rockaways. More than half of our clients are immigrants and refugees from around the globe. Over 85% of our families are living in poverty, reflecting the challenges faced by nearly 1.7 million New Yorkers today.
Program Overview CAMBA’s School-Based Early Support Program is a collaborative initiative with ACS designed to empower families and promote children's well‑being within the school environment. This program offers a range of services and supports directly accessible to families through partnerships with local schools.
Position Case Planner
Reports To Program Manager
Location 955 Flatbush Ave, 4th Floor, Brooklyn, NY 11226 — Hybrid/Remote
Responsibilities
Conduct outreach activities to engage families, including self‑referrals, and discuss program offerings and expectations, prioritizing referrals from partner schools and community‑based organizations.
Collaborate with referral sources to gain a comprehensive understanding of each family’s strengths, needs, and priorities, securing informed consent for information sharing.
Develop individualized support plans in partnership with families, based on comprehensive assessments.
Manage a caseload of 12 clients effectively.
Maintain accurate documentation (FASP reports, databases).
Prepare reports and participate in program evaluation.
Recommend case closure based on guidelines.
Participate in case conferences.
Qualifications Education and Experience
A Bachelor of Arts (BA), Bachelor of Science (BS), or Bachelor of Social Work (BSW) degree.
Minimum of two years of experience working with families and children in a community setting.
Other Requirements
Strong interpersonal and communication skills, with the ability to convey respect, compassion, and empathy to clients.
Exceptional writing skills for clear, concise, and professional documentation, reports, and communication with stakeholders.
Excellent organizational skills and ability to prioritize tasks.
Proficiency in Microsoft Office Suite.
Working knowledge of relevant databases and case management systems (a plus).
Commitment to cultural competency and anti‑oppressive practice.
Strong understanding of student support services, including IEPs, 504 plans, and SEL programs.
Bi‑lingual in English and Spanish or Haitian Creole.
Must obtain Department of Education fingerprint clearance (PETs) if working in an NYC Public School building.
If off‑site or school‑based, ability to maintain State Central Registry (SCR) clearance and fingerprint clearance throughout employment.
Compensation $47,000-$52,000 annually.
Status Full‑time (35 hours per week).
