General Summary of Position
Collaborates and consults with physicians, nurses and other healthcare professionals to ensure safe, efficacious and cost‑effective prescribing, dispensing, administration and monitoring of medication in the provision of pharmaceutical care. The pharmacist must have current specialized drug knowledge and demonstrate judgment, communication skills, self‑direction and motivation to affect positive interventions in patient outcomes beyond those exhibited by those in a Clinical Pharmacist I position. Participates in educational programs as necessary to provide basic and advanced services of the Pharmacy Department and performs role in the Clinical Pharmacist I job activities relating to drug distribution supervision and non‑dispensing functions.
Primary Duties and Responsibilities
Evaluates medication orders for accuracy and thoroughness; resolves discrepancies through communication with physicians, clinical pharmacists, or nursing staff and dispenses medication.
Assesses feasibility of compounds or verifies the compounding of extemporaneous products; recommends alternative products when appropriate.
Prepares IV admixtures and compounds sterile products, including large‑volume parenteral mini‑bags, TPN solutions, and hazardous agents such as cytotoxic drugs.
Reviews patient profiles to determine contraindications and screens drugs/dosages to assure appropriateness based on therapeutic intent and physiological functions; participates in monitoring drug therapy under supervision of a clinical pharmacist.
Dispenses controlled substances and maintains perpetual inventory; ensures legal requirements and departmental policies are consistently followed.
Provides functional supervision for support personnel, monitors attendance and work quality, organizes and prioritizes resources, and directs workflow.
Enters and verifies computer entries for drug orders; may instruct patients/families on safe medication use and counsel on potential adverse effects (decentralized setting).
Monitors drug therapy of assigned patients and takes appropriate action to ensure optimum therapy and minimal adverse reactions.
Participates autonomously in inpatient care rounds, develops therapeutic plans, and counsels/educates patients on medications.
Responds to drug information requests.
Ensures prescribers comply with clinical practice guidelines and other drug policies; minimizes non‑formulary use and eliminates therapeutic duplication.
Provides educational services to pharmacy technicians, clinical pharmacists, pharmacy residents, nurses, prescribers and other healthcare professionals.
Maintains current knowledge through literature, seminars, conferences and continuing education.
Contributes to achievement of departmental goals and adheres to policies, quality standards, safety standards and MedStar SPIRIT Values; complies with governmental and accreditation regulations.
Participates in meetings, committees, community outreach, and multidisciplinary quality and service improvement teams.
Minimum Qualifications Education
Doctoral degree from an accredited College of Pharmacy (PharmD) required.
Experience
PGY‑1 or 3 years of direct patient care experience in a hospital setting required.
Licenses and Certifications
Licensed Pharmacist (PHARMD) in the state of practice required.
Board certification within one year of eligibility required.
Knowledge, Skills and Abilities
Verbal and written communication skills; basic computer skills preferred.

USD $120,702.00 – USD $238,222.00 / year.
