licensed Physical Therapist

to provide

home health physical therapy services

across multiple counties in West Virginia. This role focuses on patient-centered care, functional rehabilitation, and improving independence. No weekends or on-call shifts are required, offering a predictable weekday schedule. Physical Therapist (Home Health) Key Responsibilities Perform comprehensive physical therapy evaluations per physician orders Review patient medical history, diagnoses, precautions, and contraindications Develop individualized treatment plans with measurable functional goals Deliver skilled physical therapy treatments in the home setting Reassess patient progress and adjust treatment plans as needed Communicate patient needs and progress with physicians, care teams, patients, and families Recommend and assist with adaptive and durable medical equipment Maintain accurate clinical and billing documentation Ensure compliance with HIPAA, company policies, and state/federal regulations Supervise Physical Therapist Assistants and Therapy Aides per state practice act Participate in staff development, training, and continuing education
