Certified and licensed Nurse Practitioner in NY.
Eligible for Medicare participation.
Minimum of 2 years of experience with complex adult populations.
Valid driver's license; comfortable with local travel.
Tech-savvy with experience in multiple electronic medical record systems.
Fluency in English; Spanish proficiency preferred.
Adaptability to a dynamic and evolving work environment.
Qualifications:
Certification in Adult, Family, Geriatric, or Acute Care.
Ability to practice independently.
Experience with predictive analytics in clinical decisions.
