Clinical Guide, Population Health RN
Rightway seeks a RN to provide clinical triage, high‑risk outreach, condition support, care planning, clinical education, and care coordination for members. Under the general supervision of the Sr. Clinical Manager, the RN will help members navigate, access, and receive the highest quality care to improve outcomes while reducing unnecessary costs. What You’ll Do
Perform clinical triage to assess the severity of members’ health concerns using evidence‑based protocols, directing care to the right healthcare resource promptly. Leverage data analytics to conduct proactive outreach to high‑risk, high‑spend, and care‑gap members, engaging them in informed decision‑making. Engage members in conversations, assess risks, preferences, and barriers, then recommend next steps that improve health outcomes. Identify and address barriers preventing members from accessing preventive care or establishing primary care relationships. Assist in scheduling appointments, referrals, and follow‑up care to close gaps in preventive and chronic disease support. Provide personalized condition education, care options, medication adherence support, shared decision support, and coordination services. Work with the Rightway clinical lead to determine the best clinical options for complex member needs. Accurately document symptoms, nursing assessment, guidance provided, and member response. Act as a health advocate, supporting members through their care journey. Coordinate completion of screening services for high‑risk members, providing adherence follow‑up and coordination support. Who You Are
RN, ADN, or BSN degree. Active, in‑good‑standing RN license in a compact state. 5+ years of direct patient care experience, preferably in family medicine. Triage nursing experience desired. Population health, disease management, or similar experience required. Care management or chronic condition support experience desired. Compact/multi‑state license; additional licenses a plus (CA, NY, OR, MI, DC). Hourly Rate
