SCO
Responsible for coordinating all intake interviews and assessments and maintaining a utilization rate of 90%.

Responsibilities
Complete pre-screenings and intake assessments for clients in need of shelter or other services.
Review assessments, psychiatric evaluations, and other documents received from referral sources.
Participate in service and discharge planning and conferencing, when appropriate.
Complete fieldwork and outreach to build a network of potential referral sources.
Report any suspected abuse or neglect to State Central Register.
Act as liaison between program sites, community based organizations, and agencies that serve runaway and homeless youth.
Complete monthly and annual statistical reports.
Maintain DYCD Connect enrollments for all new admissions.
Work collaboratively with Case Planners and administrative team to ensure quality of service.
Participate in other program activities including Program Quality Improvement.
Hire and supervise the Intake Coordinator, including ensuring the intake worker completes 40 hours per annum training requirements and performance evaluations are completed.
Provide 24‑hr on‑call intake coverage via agency cell phone and manage all emergency placements.
Any other assigned tasks.
Qualifications Bachelor's degree in a related field is required, MSW preferred.
