We are looking for dedicated primary care providers to help bring our innovative care model and compassionate approach to healthcare to the residents of San Antonio and surrounding communities.

Responsibilities

Deliver comprehensive primary care at home/senior communities visits to patients

Represent Bloom Healthcare in the San Antonio Market as a Bloom ambassador supporting our new market growth including market analysis to understand the local healthcare landscape and competitors.

Strategic planning to grow your patient panel and deliver comprehensive primary care at home visits to 10 patients per day.

Build trusted relationships by engaging with key members of our patients’ care teams, including assisted living facilities, hospitals, clinics, and community organizations, to facilitate coordination of care.

Collaborate with interdisciplinary teams, including specialists, pharmacists, and social workers, to coordinate care, address complex patient needs, and achieve optimal health outcomes.

Provide data‑driven, condition‑specific specialty visits to patients as needed

Manage the health outcomes of an empaneled patient population

Engage with and appropriately utilize population health data to drive superior patient outcomes

Attend ongoing educational opportunities

Commute to assigned assisted living communities to provide care at home

Occasional travel to Denver market for initial and ongoing training

Physician Qualifications

Appropriate Licensure in the state of Texas

Bilingual preferred but not required

The pay range for this role in the state of Texas typically falls between $225,000 - $260,000 annually with the potential for performance-based bonuses and other benefits.
