We’re different than most primary care providers. We’re rapidly expanding and we need great people to join our team.
Key Responsibilities
The PCP independently provides care for patients with acute and chronic illnesses, including older adult patients.
The PCP takes full accountability for patient care and outcomes and will appropriately seek consultation from specialists when needed.
The PCP engages with the hospitalist whenever a PCP patient is hospitalized (regardless of whether the hospitalist is a ChenMed provider).
The PCP leads their care team consisting of care promoter (medical assistant), care facilitator, and care coordinator for patients able to come to the office.
For patients that are unable to come to the office (hospital, SNF, LTC, or homebound), the PCP will engage with the transitional care team and other resources available in the market.
The PCP will have an active role in the management of their center and will help cover for other providers as needed.
Perform other duties as assigned and modified at manager’s discretion.
Qualifications
Strong clinical care, cost-effectiveness, and quality outcomes for each patient panel of up to 450 patients.
Health coaching skills: help patients set short‑ and long‑term health goals, partner with them to work toward those goals, and frequently follow up.
Ability to simplify and prioritize to make behavior change actionable for patients and to lead care teams toward performance goals.
Strong communication, leadership, and team‑building skills.
Commitment to continuous improvement of practice and center operations.
Education & Experience
MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar specialty required.
State medical license (current, active) in the state(s) of employment.
Board certification in Internal Medicine, Family Medicine, Geriatrics, or similar specialty preferred; board eligibility required.
Current DEA number for schedule II‑V controlled substances.
BLS certification from the American Heart Association or American Red Cross required within 90 days of employment.
Pay Range $221,141 - $315,915 Salary
