Under the direction of nursing leadership, the Home Health Coordinator provides essential administrative and operational support to the home care department. This role plays a key part in ensuring high-quality, compassionate home care services through effective coordination, communication, and oversight of daily operations. Duties
Facilitate and coordinate home care services for patients to ensure timely, effective support
Assist with the supervision and evaluation of caregivers
Oversee general home care operations to support quality and continuity of care
Communicate clearly and professionally, both verbally and in writing, with providers, patients, family members, and the team
Provide administrative support to the nursing department as needed
Perform other related duties as assigned
Required Qualifications
Minimum of one (1) year of relevant work experience
Prior experience working with elderly or frail populations is required
Knowledge, Skills, and Abilities
Excellent verbal and written communication skills
Demonstrated leadership ability, creativity, and flexibility
Strong interest and proven ability to work with elderly or frail patients
Ability to work independently and collaboratively as part of a team
Basic computer proficiency, including Microsoft Word and Excel
Working knowledge of basic medical terminology
Desired Qualifications
Experience supporting home care or community-based healthcare programs
Prior exposure to caregiver coordination or healthcare operations
Strong organizational and problem-solving skills
