Summary The Director of Case Management provides strategic leadership and operational oversight for the hospital case management program. This role is responsible for ensuring effective care coordination, discharge planning, and transitions of care. The Director ensures compliance with regulatory requirements, promotes patient‑centered care, and drives performance improvement across the continuum.

Required:

Master's degree in Nursing, Social Work or a related field required. Current CT Registered Nurse (RN) license. Minimum of 5 years of progressive leadership experience in hospital case management or care coordination. ACM certification required.

40 control hours Shift:

Monday to Friday 8:30 AM-5:00 PM

Responsibilities

Lead and manage a multidisciplinary team of case managers, social workers, and support staff.

Hire, train, and evaluate RN and SW Case Managers and support staff, setting performance standards and providing feedback.

Manage departmental budgets, staffing, and operational workflows to ensure efficiency and effectiveness.

Develop and implement case management strategies aligned with hospital goals and regulatory standards.

Ensure compliance with CMS, Joint Commission, and other regulatory bodies related to discharge planning, utilization review, and care transitions.

Monitor and analyze key performance indicators (KPIs) such as length of stay, readmission rates, avoidable days, discharge disposition to drive continuous improvement.

Utilize data analytics to identify trends, gaps, and opportunities for innovation in care delivery.

Lead initiatives to improve patient satisfaction, reduce hospital readmissions, and enhance care coordination.

Collaborate with clinical and administrative leaders to optimize patient flow and resource utilization.

Oversee the development of individualized discharge plans that ensure safe and timely transitions of care.

Serve as a liaison with external agencies, payers, and community partners to support continuity of care.

Ensure timely and accurate documentation in accordance with hospital policies and regulatory requirements.

Participate in hospital-wide quality improvement initiatives and strategic planning efforts.

Develop and maintain policies and procedures for the case management department.

Support risk management and compliance teams in audits, surveys, and corrective action planning.

Provide education and training to staff on case management best practices, regulatory updates, and care coordination models.

Foster a culture of accountability, collaboration, and excellence within the department.

Collaborate with IT and data teams to ensure accurate reporting and integration of case management systems.

Represent the department in executive meetings, committees, and community outreach efforts.

Perform other duties as assigned.

Other Information Strong knowledge of healthcare regulations, managed care principles, and accreditation standards. Proven ability to lead teams, manage change, and drive performance improvement. Excellent communication, analytical, and organizational skills.
