Doctorate of Pharmacy - Required
Experience 2-3 Years Clinical Ambulatory Care practice post residency/fellowship - Preferred
Prior experience in teaching/faculty - Preferred
License & Certification Professional License with State of Practice - Required
Addition to PharmD and residency -board certification in a pharmacy specialty within the first 3 years - Required
Core Job Functions
Assess pharmacotherapy and individualize therapy to specific patient needs and communicate therapeutic recommendations to the Medical Staff. Establish and clarify goals and objectives of therapy in collaboration with the physician for each problem being treated. Provide patient education and counseling and participate in discharge planning for the purpose of ensuring continuity of drug therapy, avoiding adverse drug reaction, and reducing waste.
Provide consistent and reproducible evidence based preventative medical recommendations to patients of SJ/C Physician Network or as referred. Maintain active Certificate License of Drug Therapy Modification with supervising physician(s) with the Georgia Board of Pharmacy.
Follow College of American Pathologists (CAP) protocols regarding Clinical Laboratory Improvement Amendments (CLIA) requirements for proficiency testing for departments of the hospital. Maintain documentation of quality control (QC) tests completed.
Document timely and efficiently in eClinicalWorks electronic medical record platform. Apply templates as assigned.
Develop and implement clinical pharmacy training programs. Participate in the education and training of technical staff, pharmacy residents, pharmacy interns, and graduate pharmacy students. Assist in the design, implementation and management of clinical pharmacy programs throughout the organization.
Maintain organization under the scope of clinical practitioners. Integrate and advance student and resident professional development and patient care skills. Participate in the orientation and training of new staff.
