Job Description The Clinical Pharmacist Central Operations is a licensed pharmacist responsible for ensuring optimal drug therapy by designing therapy, monitoring, and dispensing medications following standard operating procedures (SOPs), ACLS, PALS, and USP guidelines.
Serves as medication expert, directs support staff including Pharmacy Technicians, and ensures medication management abides by hospital, state, and federal requirements.
Manages the daily workload within the central pharmacy and is able to cross‑cover as the Pharmacist across all hospital departments as needed.
Also covers in the IV room as needed.
Responsibilities also include, but are not limited to: answering general drug questions, attending medical rounds, completing nursing unit inspections, and identifying medication‑related issues.
May function as a Charge Pharmacist as needed.
Super user of pharmacy related software and/or automation.
Responsibilities
Verifies and checks prescriptions/orders adhering to safe medication dispensing practices.
Reviews prescription orders for accuracy and completion.
Provides medication evaluation during verification, considering patient‑specific factors, potential for adverse drug reactions, and Drug‑Drug / Drug‑Disease interactions.
Assesses and optimizes drug therapy by designing medication therapies that meet patient specific needs.
Develops, reviews, assists, and maintains protocols and regimens in collaboration with pharmacy leadership and other departments, based on evidence‑based practice.
Utilizes the facility formulary whenever possible to ensure cost‑effective care.
Provides drug information education to patients or caregivers as needed.
Mentors and assists in training pharmacists, interns, residents, and other inpatient pharmacy staff.
Teaches and instructs pharmacy and multidisciplinary staff on relevant treatment populations, including preparation, treatment, and ongoing monitoring of medications for a broad range of disease states.
Provides perpetual monitoring of assigned patients through chart investigation, discussion with the healthcare team, and remediation of drug therapy.
Rounds with the medical team as needed.
Manages the daily pharmacy workload within the inpatient pharmacy.
Supervises pharmacy staff throughout the shift (technicians, students, residents, interns) to ensure safe and accurate preparation, dispensing, and delivery of medications.
Keeps detailed documentation of medication monitoring, management, adverse drug reactions, and interventions in the electronic health record (EPIC, DoseEdge, Omnicell).
Maintains documentation compliant with state and federal drug control laws.
Inspects medication storage and prep areas outside of the Pharmacy monthly, raising concerns to Nursing and Pharmacy leadership.
Identifies and escalates medication or operational issues to direct manager.
Contributes to adding new products to the formulary through the P&T committee, developing presentations, education, and monographs.
Performs other job‑related duties as assigned.
Requirements
BS degree in Pharmacy and PharmD.
Completion of PGY1 Pharmacy Practice Residency; completion of PGY2 Pediatric Pharmacy Practice Residency preferred.
State of Connecticut Pharmacist License.
Board certification in an area of specialty or advanced training/certification preferred.
BLS certification.
CITI training to cross‑cover investigational studies related to Oncology.
Knowledge, Skills, and Abilities
Medication doses, indications, monitoring parameters, risks, contraindications, drug‑drug and drug‑disease interactions.
Medication preparation and delivery processes.
Common disease states affecting pediatric populations and their risks and treatments.
Critical thinking and problem solving.
Utilizing available resources such as medication and disease‑related guidelines.
Teaching and learning.
Appropriately assessing and managing competing priorities.
