Family Practice Physician
Chicago Family Health Center seeks a highly skilled Family Medicine Physician to provide primary care services in underserved communities.
Overview Deliver comprehensive, evidence‑based primary care in office and community settings, collaborate with interdisciplinary teams, and promote the mission of the Medical Department.
Responsibilities
Provide comprehensive primary care to patients in office, hospital, or community settings.
Consult with clinical management on goals and services.
Manage patient care collaboratively with clinical staff and care team.
Apply industry knowledge and best practices to support department vision.
Support culture development and management initiatives.
Qualifications
M.D. or D.O. from an accredited allopathic or osteopathic medical school.
Board‑certified or board‑eligible in Family Practice.
Current Illinois medical and DEA licensure.
Hospital privileges at affiliated hospitals.
Bilingual ability may be required based on department needs.
Knowledge, Skills & Abilities
