Description

TITLE: Aftercare Worker

SUMMARY: The Aftercare Worker works to support families who have been successfully discharged from DHS/CUA formal case-management services by providing reintegration supports, consultations and community resources that will decrease the probability that the family will re-enter the child welfare system. This position involves working closely with all members of the assigned case management team prior to successful discharge; with children, biological families, and with external agencies and partners. The Aftercare Worker will focus on planning intended to prevent re-entry. Tasks will include direct work with families to address needed ongoing community supports after formal DHS/CUA case-management services are discharged, as well as work with external partners and systems to ensure that children and families are successfully connected to needed reintegration and support services.

Qualifications

MINIMUM QUALIFICATIONS: Bachelors Degree in Social Work or related field required. Candidates with a degree in an unrelated field require a minimum of three years experience working in child, youth, and family serving systems. Must have strong interpersonal skills and the ability to communicate well orally and in writing. Must have a valid drivers license.
