SUMMARY:
The Medical Care Coordinator (LPN) plays a critical role in supporting clients with complex medical and behavioral health needs at Northeast Treatment Centers' Certified Community Behavioral Health Center (CCBHC). The coordinator is responsible for assessing individual physical health needs through screenings, interviews, and review of medical records to develop tailored care coordination plans. These plans include internal services and external referrals to ensure holistic, client-centered care.
Serving as a key member of the integrated care team, the Medical Care Coordinator provides disease management education, monitors medication adherence, and assists clients in overcoming barriers to care. Additionally, this position advocates for clients navigating the healthcare system and fosters continuity of care through collaboration with medical providers and behavioral health professionals.
Qualifications
MINIMUM QUALIFICATIONS
: Graduate of a Board-Certified LPN Program. Current PA LPN Licensure in good standing. At least 1 year LPN experience Knowledge of mental health and substance abuse systems and supports Ability to work independently Strong organizational skills Ability to and comfort with working with individuals who have serious mental illness and/or substance abuse
