Job Description:
The Director of Quality & Performance Improvement (QAPI) is responsible for building and sustaining the organization’s Quality Assessment and Performance Improvement program in accordance with federal and state regulations. This leader establishes the quality infrastructure, defines key performance metrics, drives continuous improvement initiatives, and ensures survey readiness across both hospice programs.
Responsibilities
Develop and implement the agencies’ QAPI plan aligned with CMS Conditions of Participation and Texas hospice regulations.
Create audit tools, data dashboards, and reporting systems that ensure accurate tracking of performance and compliance metrics.
Identify trends, analyze root causes, and lead measurable improvement initiatives that improve patient care and operational performance.
Partner with clinical leadership to implement corrective actions, ensuring consistent follow-through and accountability.
Maintain agency survey readiness at all times; oversee preparation and participation in state, federal, and accreditation surveys.
