Job Description Job Description We are seeking Quality Experienced CNA, HHA's for Home Care visits and shifts, to become a part of our team! Assignments throughout Hillsborough county. Responsibilities: Provide routine individualized care to the elderly, convalescents, or persons with disabilities Monitor and report changes in patient health status Provide personal care, such as bathing and dressing patients, ambulation, light housekeeping & meal prep Follow Patient Care Plan as directed by Nursing Supervisor Qualifications: Experience in hospice, home health, or other related fields Positive and caring demeanor Ability to build rapport with patients and family members Excellent written and verbal communication skills in English Must be Fluent in English speaking and documentation Must have: Level 2 Background screening, BLS-CPR, HIV, Self Administration of Medication & 2 Hour Alzheimer's and Related Dementia Certificates, 1 HR- Elder Affairs Alzheimer Certificate.
